Overview

In February 2006, the Council of Australian Governments (COAG) announced a four-year, national program called the Australian Better Health Initiative (ABHI) to strengthen the health system’s focus on promoting good health and reducing the burden of chronic disease. One of five elements in the ABHI initiative is the Primary Care Integration Program (PCIP). The Australian Government has provided funding to the General Practice Tasmania Network via the ABHI Primary Care Integration Program.

 

Objectives

The overarching national aim of the Program is to promote solutions to primary care integration between general practice and other local health providers that will assist in delivery of more 'seamless' patient care. This is particularly important in the context of better managing patients with chronic or complex conditions who often receive care from multiple providers, funded by different sources, across different settings.

The objective of this Program is to encourage more integrated patient centred care by supporting general practice across Tasmania to:

  • Communicate and link better with other primary care providers;
  • Utilise existing primary and community care services including Commonwealth, State and non-government organisations funded services that are targeted at patients across the whole chronic disease continuum;
  • Utilise tools/strategies that will assist in better managing patients with chronic disease (e.g. disease registers, recall & reminder systems);
  • Contribute to work around developing local chronic disease care pathways (generic or specific) and referral tools; and
  • Engage with State funded primary care initiatives, particularly, implementation of the Tasmanian Primary Health Services Plan (PHSP).

To achieve these objectives the SBO has been funded to provide statewide leadership, coordination and oversight of ABHI Primary Care Integration Program within Tasmania, and to execute and manage sub-contracts with its three member Divisions of General Practice.

Achievements

Development of a whole of General Practice Network workplan in collaboration with the State Department of Health and Human Services.

Development of a position statement regarding the implementation of the GP Super Clinics Initiative in Tasmania.

Planning for the implementation of a diabetes demonstration service to be based in Launceston.
