Overview
General Practice Tasmania is funded to work with the Pharmacy Guild of Australia and the Tasmanian Divisions of General Practice with the implementation of the Medication Management Review (MMR) Facilitator Program. Divisional Facilitators assist both general practice and pharmacy take up the opportunity that HMRs represent to provide evidence-based, multi-disciplinary and preventive care to consumers.
Australia's National Medicines Policy is a cooperative endeavour to bring about better health outcomes for all Australians, focusing especially on people's access to, and wise use of, medicines. Medication Management Review (MMR) as part of that framework aims to improve Quality Use of Medicine (QUM) in the community.
The Home Medicine Review (HMR) was introduced into the Medical Benefits Scheme in 2001 with Residential Medication Management Review (RMMR) being introduced in 2004.
Program Objectives
The HMR program aims to bring together the consumer, their GP, community pharmacy and an accredited pharmacist as a team, with RMMR the team expands to include the Residential Aged Care Facility staff. The goal of both HMR and RMMR is to:
- Achieve safe, effective, and appropriate use of medications by detecting and addressing medication related problem/s that interfere with desired patient outcomes
- Improve the patient's quality of life and health outcomes using a best practice approach, that involves a collaborative effort between the GP, pharmacist, other relevant health professionals, and the patient (and where appropriate, their carer)
- Improve the patient's and health professionals' knowledge and understanding about medications
- Facilitate cooperative working relationships between members of the health care team, in the interests of patient health and well-being.
To progress these program objectives General Practice Tasmania:
- Works with the Pharmacy Guild of Australia and Divisions to support program implementation and meeting of MMR Facilitator Program outcomes.
- Improves inter and intra state networks and linkages between Divisions, the Pharmacy Guild of Australia and other stakeholders.
- Encourages information sharing and collaboration between Divisions.
- Maintains regular liaison and support for all Division HMR Facilitators and the Guild State Coordinator.
- Develops and maintains strategic partnership opportunities with other relevant organisations and professional bodies.
- Collaborates with National Prescribing Service Program in Divisions where appropriate on QUM issues
